Local Pharmacies Connecting Community and Patients
As trusted community health providers, CPESN community pharmacies are on the front lines every day with patients confronting health-impacting inequities. Integrating Community Health Workers, navigators, and peer health mentors to the pharmacy can expand attractive service offerings funded through a variety of local, state, and federal funding sources, as well as public-private payer partnerships.
Increasingly, Medicaid MCOs and exchange payers are adding social determinants of health screenings and referrals to community pharmacy payment models. Local pharmacy support staff - including delivery drivers, and cashiers along with technicians and CHWs - have the trust and touchpoints to identify barriers and solve problems. For health plans, that can lead to cost-saving mitigation.
