Integrating Health Equity Into Community Pharmacies

CPESN Health Equity helps pharmacies create a new line of revenue – health equity revenue for the delivery of services to identify, resolve, and treat health-related disparities. Care Coordination –  During routine pharmacy visits, the CHW or SDoH Specialist integrated within a CPESN pharmacy provides care coordination services, and access to add-on chronic disease management services as needed. 

340B Readiness – Health Equity helps CPESN become market ready to participate in 340B opportunities HE experts team with the pharmacy and Federally Qualified Health Center (FQHC) to build the optimal 340B model for your unique partnership.
