Integrating Health Equity Into Community Pharmacies
CPESN Health Equity helps pharmacies create a new line of revenue – health equity revenue for the delivery of services to identify, resolve, and treat health-related disparities. Every affiliated pharmacy has social determinants of health expertise while training at least one Community Health Worker - expanding roles of technicians, support staff, and delivery drivers to help patients with high social risk navigate healthcare and community resources.

Bridging Gaps, Enhancing Care
With more than 300 affiliated pharmacies in 30+ states, CPESN Heath Equity is the largest network of pharmacies committed to providing pharmacy care with a special emphasis on underserved and marginalized patients. Increasingly, Medicare, Managed MCOs, and state Medicaid programs are recognizing these services in value-based payment models. CPESN Health Equity helps CPESN pharmacies and local networks navigate conversations with health departments, state Medicaid agencies, and value-based payers around best-practice services to address health inequities.
Expand Access, Add Revenue with Health Equity Services
Care Coordination – During routine pharmacy visits, the CHW or SDoH Specialist integrated within a CPESN pharmacy provides care coordination services, and access to add-on chronic disease management services as needed.
340B Readiness – Health Equity helps CPESN become market ready to participate in 340B opportunities HE experts team with the pharmacy and Federally Qualified Health Center (FQHC) to build the optimal 340B model for your unique partnership.
Work Force Development (CHW training) – Health equity development program for pharmacy employees, including Community Health Worker training, and staff training in SDoH expertise.
Grant Facilitation – HE can assist pharmacies to identify, obtain, or execute on government and private grant opportunities.
Vaccine Gap Closure – HE has a strong record of implementing successful pharmacy-led programs to close vaccine gaps and drive clinical service revenue to participating pharmacies.
Opioid Education – HE can teach community pharmacies how to deliver effective opioid education programs and demonstrate outcomes to local governments seeking help to reduce the loss of life due to overdoses.
Medication Optimization – Help pharmacies develop a team-based approach to managing medication therapy to help patients better understand and maximize prescribed medications.
Self-Monitoring Blood Pressure – Drive new pharmacy revenue with a program that teaches patients to self-monitor and manage their elevated blood pressure
